The identification of robust prognostic markers for ovarian cancer will increase the likelihood of success with therapeutic intervention. We used a microarray-based approach to examine the pattern of gene expression in tumor tissue from 170 ovarian cancer patients to assess its association with clinicopathological features, long-term survival, and the patterns of relapse. The use of unsupervised hierarchical cluster analysis revealed four main clusters. Cluster 1 (n = 80), in which the majority of patients showed favorable prognosis, was significantly different from all other clusters. Gene-expression profiling of cluster 1 tumors identified 61 transcripts differentially expressed in this prognostic group. Validation of microarray data by reverse transcription-polymerase chain reaction analysis confirmed the differential expression of a number of these genes. In this cluster, the expression of eight genes (PGF, IGFBP5, Maspin, C5orf17, MKL2, EGR2, ER, and CK5) was significantly associated with patient survival. A classification model based on the expression of these eight genes was able to distinguish between patients with favorable and poor prognosis with a sensitivity of 75% and a specificity of 87%. Analysis of patients in cluster 1 indicated that the expression of IGFBP5 was significantly associated with both tumor grade and stage. In addition, the expression of IGFBP5 was associated with patient survival. This study provides strong evidence that the molecular profile of ovarian cancer is associated with both patient prognosis and tumor grade and stage and that a combination of these biomarkers could improve patient risk stratification.Hawksnest
